
There are some incidents we don't think about because avoiding them is obvious: runway incursions. These are "any occurrence at an airport including an aircraft, vehicle, person, or object on the ground that creates a collision hazard or results in a loss of separation with an aircraft taking off, intending to take off, landing, or intending to land." How, at a towered airport, could one get onto an active runway without a clearance? Why? I mean, like, we know the regs; besides, ground control will watch over us, right?
With my ego, I like to think that I, a supreme aviator, am more skillful, safety-conscious, knowledgeable, and careful than most aviators out there. (Whether I really am is another question.) This malady, known as "complacency," is common to people who are pilot examiners, write columns, run flight schools, and have much experience. We fall into this trap and think we're immune to human frailties. But it seems that no sooner do I start thinking this that something happens to return me to the real world.
Recently, I landed for fuel at an airport where I'd been before, but it'd been awhile. It had changed, construction in progress, with several closed runways and taxiways. It was a Class C airspace airport with two intersecting runways. The construction required long taxis but it wasn't so bad that someone with my extensive experience and obvious skill couldn't handle it.
Taxiing in was uneventful, dodging construction equipment and torn up areas. Departing, however, my brain failed, even though I'd studied the airport diagram. The taxi directions included crossing an active runway (with a "hold short" provision) and a circuitous route. While relating my taxi route to the airport diagram, I let my airplane roll past the hold line onto a runway, forcing one aircraft to go around; about the first third of my Cessna 180 was on the runway.
Why didn't I get a progressive taxi? - two reasons. First, with my extensive aviation experience and obvious skill, why show my ignorance? I mean, like, c'mon, that's uncool. Second, the local controller and ground controller were the same person and he was busy with other traffic, both ground and air. With my extensive skill and experience, I couldn't interrupt.
As we all know, every occurrence has a chain of events leading to it. With a mishap, these events constitute an
"accident chain," which if any one link were broken, there would probably have been no mishap. Runway incursions, like other incidents, have a number of events leading to them and incursions themselves can be an accident chain link.
There are four parts to preventing incursions: (1) clearances, (2) communications, (3) ground navigation, and (4) situational awareness, including scanning. If any one of these fails, the incursion probability increases; if more occur, then they're inevitable.
Why is each of these components critical? Well, for those of us who fly from busy airports, with a GA, corporate, and airline traffic mix, maybe including military, think about what can happen if the system fails. The recent unpleasantness in Quincy, Illinois, is one example, and those who've been around a few years recall the airliner landing atop a commuter at LAX. These are two catastrophic examples of what can happen; as we all are human and make mistakes, it takes everyone's effort prevent them.
So let's look at each of these. First, clearances: pilots must understand what they're told to do. If they don't, or can't comply, then get a clarification or an amended clearance!
Second, communications: use proper procedures, standard words and phrases, and read back your clearance, particularly if it's complex or if you're unfamiliar. If appropriate, request a progressive taxi.
Third, ground navigation: know the airport layout before engine starting; use that airport map on your approach chart or in a directory. Know airport signage, especially now that it's standardized. The AIM has color pictures (imagine that!) showing these signs; they're easy to learn and more intuitive than before.
Fourth, situational awareness, including scanning: brief, then use, passengers to monitor your taxi. Monitor your own progress, too; if you, or your passenger, has a question about what's happening, ask! Use all your resources, including ATC, to help. Scan for other traffic, including airplanes (and helicopters) landing or taking off, and watch for pedestrians or vehicles not where they should be. Avoiding incursions is a team effort between controllers (coordinating traffic), pilots (responsible for aircraft safety), and ground personnel.
Oh, by the way, what happened to me? Did I get
a violation? Are examiners immune to prosecution? Well, not this
particular examiner. Did I receive a letter of investigation?
Yes. Was I found guilty? Yep. What happened? A warning letter
that'll be in my file. Did I file a NASA ASRS report? You betcha!
Will I be more careful in the future? What do you think? (Don't
answer that!)
